FORMS & APPLICATIONS

Must submit a permit fee with all applications and include the following:

 

  • General Liability Insurance Certificate (accord form)

  • Workers Compensation Certificate (U-26.3 state form) (issued by the NYS Insurance Fund)

  • Disability Insurance Certificate

  • Copy of Contractors Nassau County or Town License

 

**ALL CERTIFICATES MUST LIST  “Village of Russell Gardens, 6 Tain Drive, Great Neck, NY 11021” as the Certificate Holder & Additional Insured.
